In a large mixing bowl, combine the ground turkey, uncooked rice, chopped mushrooms, chopped onion, kosher salt, and black pepper and mix gently until evenly distributed.
Shape the mixture into 20 meatballs, each 1 1/2 inches in diameter and approximately 2 oz each.
Arrange the meatballs in a single snug layer inside the slow cooker.
In a separate bowl, whisk together the tomato sauce and 1 cup water until fully combined.
Pour the tomato mixture evenly over the meatballs, ensuring they are mostly submerged.
Cover with the lid and cook on LOW for 6 hours or HIGH for 3 hours until the internal temperature reaches 165°F and the rice is tender.
Let rest covered for 10 minutes before serving.
Serve 4 meatballs per portion with approximately 1/2 cup tomato sauce.
