Lay 4 sheets of heavy-duty aluminum foil on the counter.
Lightly crimp the edges upward to help hold the sauce.
Whisk together the honey and Italian vinaigrette until smooth.
Pat the pork chops dry.
Divide the baby carrots evenly among the foil sheets.
Place one pork chop on top of the carrots in each foil sheet.
Spoon the honey vinaigrette mixture evenly over the pork chops and carrots.
Bring two opposite sides of each foil sheet together over the pork chop.
Fold tightly several times to seal.
Fold and crimp the remaining sides to make snug packets.
Place the packets seam side up in the slow cooker.
Cover and cook on LOW for 4 to 5 hours, until the pork chops are tender and the carrots are soft.
Carefully remove the packets with tongs.
Open each packet away from your face.
Spoon the glaze from the bottom of each packet over the pork and carrots.
