Lightly spray the inside of a 5- to 6-quart slow cooker with nonstick cooking spray.
In a large skillet over medium heat, cook the ground beef for 6–8 minutes until browned and no longer pink, breaking it apart as it cooks and draining excess grease.
Transfer the cooked beef to the slow cooker and stir in the sloppy joe sauce until evenly combined.
Add the dry elbow macaroni and water, stirring until the pasta is mostly submerged.
Cover and cook on LOW for 2 hours, stir once, then continue cooking up to 3 hours total until macaroni is tender.
Let sit for 5 minutes before serving.
